  • Positive PPD (needs the Following): A) Written documentation by Doctor with history of Positive PPD B) Negative Chest X-Ray Report.
  • Rubella Immunity Proof (lab Printed Report).
  • Rubeola Immunity Proof of (Lab Printed Report).
  • Drug Screen (Lab Printed Report).
  • Proof of Flu Vaccine for the season (if you did get it).
  • Direct Deposit form, Voided check or Routing & Bank account numbers.
